Submit Online Claiming Provider Agreement (HW027)

Notify Services Australia of your Minor ID — send directly from Light or download to submit manually

You must submit the Online Claiming Provider Agreement (HW027) to register your Minor ID with Services Australia. This is required before you can submit vaccination encounters to AIR.

Light pre-fills the form from your site profile and can send it directly to Services Australia.

Step 1: Open the Form

  1. Go to Settings → Integrations in Light
  2. On the AIR Integration card, click the dropdown menu
  3. Select Online Claiming Provider Agreement (HW027)

Step 2: Review Pre-Filled Details

Light fills in the following from your site profile:

  • Provider Number, RA Number, Minor ID
  • Organisation name, address, phone, and email

Review and correct any details that are out of date.

Step 3: Complete Remaining Fields

  • Your Details — title, family name, first given name
  • Postal Address — expand if different from organisation address
  • Bank Details — expand if needed for payment setup

Step 4: Sign the Declaration

Draw or type your signature using the signature pad. This is embedded into the PDF.

Step 5: Submit

Click Send to email the form directly to Services Australia.

  • Sent to [email protected]
  • A copy is BCC'd to your site email
  • A PDF is also downloaded for your records

Your site must have an email address configured in its public profile for Send to work.

Click Download to generate an editable PDF you can review or submit manually.

What Happens Next

Services Australia processes registrations within 2–5 business days. Once confirmed, you can begin submitting vaccination encounters to AIR.
