Recording Medical Contraindications
Record permanent and temporary medical contraindications to vaccines
Record a medical contraindication to document a medical reason why a patient cannot receive a specific vaccine. Contraindication records are submitted directly to AIR.
When to Record
- Documented allergic reaction or anaphylaxis to a vaccine component
- Medical condition that contraindicates vaccination
- Severe adverse reaction to a previous dose
- Medical practitioner determination
Only record contraindications with proper clinical justification. Do not record for patient preference, vaccine hesitancy, or temporary illness that will resolve in days.
How to Record
- Open the patient's profile (must have green AIR badge)
- Find the Medical Exemptions card
- Click the action to record a new contraindication
- Complete the form:

Form Fields
- Vaccine (required) — select the contraindicated vaccine from the dropdown
- Type (required) — Permanent (no end date) or Temporary (with end date)
- Reason (required) — medical reason for the contraindication
- Start Date (required) — when the contraindication began
- End Date (required for temporary) — when the temporary contraindication expires
- Anaphylaxis Date (optional) — date of anaphylaxis event if applicable
- Review all details and click Submit
Temporary contraindications have an end date after which the patient may be eligible for vaccination. Schedule a catch-up date to review after the contraindication expires.
